The Affordable Care Act (ACA) added sections 6055 and 6056 to the Internal Revenue Code, requiring applicable large employers (ALEs) to report the following information on an annual basis:

1. The number of full-time employees (FTEs) for each calendar month of the year
2. The months employer-sponsored minimum essential coverage (MEC) was offered to each FTE and their eligible dependents during the calendar year
3. The employee's share of the lowest cost monthly premium for self-only coverage providing minimum value
4. The calendar months an employee and their eligible dependents were enrolled in a self-insured employer-sponsored medical plan
